Payment must be received in full immediately after services are provided. A "superbill" receipt will be available for your records, or to submit to your insurance company yourself. No other document(s) will be provided.

Insurance Billing
We bill patient's insurance company at the usual and customary fee schedule.* Payment from the insurance company will be made directly to Emily Rucker, D.C. Copays are to be paid at the time services are rendered. Patient will be billed for, and agrees to pay for, any additional percentage or for non-covered services, per your insurance agreement.

* The usual and customary fee schedule is a fee schedule developed by the insurance industry to determine reasonable reimbursement for Chiropractic services. Each insurance company has their version of a usual and customary fee schedule, and will adjust the fees accordingly.

Missed Appointment Fee
If you must cancel or reschedule an appointment, a 24-hour notice is appreciated. We realize that it is sometimes impossible; reasonable cause will be taken into consideration. Repeated failure (your first miss is a pass) to provide at least 4 hours cancellation notice will result in a cancellation charge of $25.00. This charge is not billable or reimbrusable by insurance.
